Any second edition has, to some degree, bear comparison with its predecessor. In this case, the second edition of a classic text has a considerable task which it accomplishes very impressively. The first edition appeared almost twenty years ago, and although at first glance those two decades may not seem to have been the most revolutionary in the history of psychotherapy there have been immense strides in the quality and nature of the evidence to support, and in some cases critique different models of intervention. This is where some of the most interesting new material can be found. The advances in brain imaging and neurochemistry have given psychotherapies a new and very exciting focus. In the new edition there are updates that reflect both the continuity and change in psychotherapy (note the subtitle) and many of these are now augmented with deeper considerations of research, practice, and training. Each chapter highlights the historical roots, current practice, and future directions. However, there several entirely new chapters that deal with issues of multiculturalism and what it might mean for what might be seen as a culturally bound phenomenon, neuroscience, couples therapy, pharmacotherapy, spirituality and religion, and specialized research. The emphasis on evidence as a foundation of practice is particularly noteworthy. And it is to the credit of both authors and editors that where the evidence is not strong, the issue is not shirked. There are some frank passages that clearly indicate that there is still work to be done, or in a historical context, that not enough work was done to sustain the scientific credibility of the approach. There are 18 chapters with contributions from 82 authors – some old, some new and overwhelmingly American (although that last point only really becomes intrusive in the final chapter that deals with education and training, PhD and PsyD, Internships and licensing and so on). The scholarship is impressive throughout. Each chapter and contribution is massively referenced, well presented and fits within the editorial structure. Both these topics could be controversial and could be presented in highly partisan ways, but that is not the case. While PTSD appears in a number of different chapters, with the evidence being weighed carefully and even-handedly in each case, spirituality is covered in depth by Everett Worthington, well known also for his work on forgiveness. Here we find not proselytizing, but a concern for meaning-making.
